Autism spectrum disorder (ASD) is a neurological developmental disorder with a broad spectrum – from high-functioning autism to Asperger’s syndrome. What CBD can help with autism
Behavioral problems and self-harm: Aran et al. shows the strongest effects in severe behavioral problems (aggression, self-harm). CB1 in the limbic system modulates impulsivity and emotional dysregulation.
Social anxiety: CBD has an anxiolytic effect via 5-HT1A. Social anxiety is one of the most common ASD comorbidities. fMRI data (Pretzsch 2019) show CBD-induced normalization of limbic responses to social stimuli.
Sleep disorders: 80% of ASD children suffer from sleep problems. CBD (10-25 mg in the evening) can shorten the time it takes to fall asleep without REM suppression.
Sensory hypersensitivity: TRPV1 desensitization by CBD can reduce tactile and acoustic hypersensitivity – mechanistically plausible, clinically not yet systematically investigated.
THC for autism: caution
While CBD is increasingly considered safe for ASD, THC is more problematic:

– Psychotic symptoms are more likely to be triggered by THC in ASD patients (especially with a family history of psychosis)

In Germany, cannabis is available on prescription for children and adolescents, but the hurdle is higher:
– Pediatric neurologist or psychiatrist must provide the indication
– Detailed documentation: previous therapies (risperidone, aripiprazole), risk-benefit assessment
– Usually CBD-rich strains (THC <0.5%); dronabinol for children rare
