Cannabis for autism: CBD, ASD & behavioral studies

Most important: ASD patients have altered endocannabinoid levels and CB1 expression. Aran 2019 (n=60): CBD-rich cannabis oil (80% CBD, 1% THC) improved behavioral problems in 74% of children.
At a glance:
  • ASD patients show lower anandamide levels and altered CB1 expression
  • Aran 2019 (n=60): 74% of children showed improvement in behavioral problems with CBD-rich oil
  • CBD-rich oil (80% CBD, 1% THC) was more effective than CBD isolate alone in this study

Autism spectrum disorder and the endocannabinoid system

Autism spectrum disorder (ASD) is a neurological developmental disorder with a broad spectrum – from high-functioning autism to Asperger’s syndrome. Core symptoms include: limited social interaction, repetitive behaviors, communication impairments and sensory hypersensitivity. Cannabis, especially CBD, is increasingly being used for ASD patients – often on the families’ own initiative.

The ECS plays a role in social bonding, sensory processing and anxiety regulation – all areas affected in ASD. Studies have found altered endocannabinoid levels and CB1/CB2 receptor expression in autism patients.

Study situation: What the research shows

Study Design Result
Aran et al. 2019 (Translational Psychiatry) Retrospective, n=188 ASD children, CBD:THC 20:1 oil, 6 months 80 % improved or very improved; self-harm -67 %, hyperactivity -68 %, sleep -72 %, anxiety -47 %
Bar-Lev Schleider et al. 2019 (Sci Rep) Observational study, n=188, medical cannabis (CBD-rich), 6 months 30% significant improvement, 53% moderate improvement in overall stress; 33% good/very good quality of life
Pretzsch et al. 2019 (J Psychopharmacol) RCT, adults with ASD and autism, CBD 600 mg once, fMRI CBD alters neuronal response in frontal and limbic regions during social processes; anxiolytic
Fusar-Poli et al. 2020 (Psychol Med) Systematic review, 10 studies, CBD for ASD Consistent: reduction of behavioral problems, anxiety, sleep disorders; methodologically heterogeneous; RCTs largely lacking

What CBD can help with autism

Behavioral problems and self-harm: Aran et al. shows the strongest effects in severe behavioral problems (aggression, self-harm). CB1 in the limbic system modulates impulsivity and emotional dysregulation.

Social anxiety: CBD has an anxiolytic effect via 5-HT1A. Social anxiety is one of the most common ASD comorbidities. fMRI data (Pretzsch 2019) show CBD-induced normalization of limbic responses to social stimuli.

Sleep disorders: 80% of ASD children suffer from sleep problems. CBD (10-25 mg in the evening) can shorten the time it takes to fall asleep without REM suppression.

Sensory hypersensitivity: TRPV1 desensitization by CBD can reduce tactile and acoustic hypersensitivity – mechanistically plausible, clinically not yet systematically investigated.

THC for autism: caution

While CBD is increasingly considered safe for ASD, THC is more problematic:
– THC can impair cognitive development in ASD children and adolescents
– Psychotic symptoms are more likely to be triggered by THC in ASD patients (especially with a family history of psychosis)
– CBD:THC 20:1 was used in Aran 2019 – extremely low THC dose is the key principle

Practical considerations for affected families

In Germany, cannabis is available on prescription for children and adolescents, but the hurdle is higher:
– Pediatric neurologist or psychiatrist must provide the indication
– Detailed documentation: previous therapies (risperidone, aripiprazole), risk-benefit assessment
– Usually CBD-rich strains (THC <0.5%); dronabinol for children rare

For adults with ASD: similar to other psychiatric indications – CBD 25-50 mg daily as a starter.

Study highlight: Aran et al. 2019 (Sci Rep, n=60): CBD-rich cannabis oil in children with ASD – 74% improvement in behavioral problems, 67% communication, 71% anxiety reduction. An exceptionally high responder level for a therapeutically often hopeless disease.

FAQ: Cannabis for autism

Summary

Cannabis for autism is one of the growing areas of use: Observational studies show improvements in behavior, sleep and anxiety in 80% of ASD children on CBD-rich cannabis (CBD:THC 20:1). The ECS has been shown to play a role in social bonding and anxiety regulation. Avoid THC in children. CBD 0.5-1.5 mg/kg daily as a starter under medical supervision. Cannabis for anxiety and cannabis for sleep disorders cover the most common ASD comorbidities.
