Cannabis and the Heart: Cardiovascular Risk, THC and Heart Rate
Cannabis and the heart — a topic that often goes unnoticed in public discussions. While most users worry about the lungs or psyche, the cardiovascular effects of THC are acutely relevant and potentially dangerous for certain groups. At the same time, CBD shows a significantly more favorable cardiac profile. What cardiology knows and who needs to be especially cautious.
THC and Heart Rate: Immediate Effects on the Cardiovascular System
Cannabis consumption triggers measurable cardiovascular reactions within minutes:
- Tachycardia as an immediate reaction: THC increases heart rate by 20–100% compared to the resting pulse within 10–15 minutes — dose-dependent. Mechanism: CB1 activation in the central nervous system and sympathetic nervous system → increased noradrenaline release. At the same time, vasodilation of peripheral vessels → drop in blood pressure. Compensatory increase in heart rate
- Duration of increased heart rate: Inhaling: Tachycardia peak after 10–30 minutes, decrease after 1–2 hours. Oral (edibles): delayed, longer-lasting effect — tachycardia possible over several hours
- Vasodilation: THC lowers blood pressure through peripheral vasodilation (CB1 on smooth muscle cells of blood vessels). Orthostatic hypotension can occur when standing up — risk of fainting, especially with first-time use
- Chronic use and adaptation: With regular use, tolerance develops against cardiovascular effects. Increases in heart rate and blood pressure reactions become significantly smaller — CB1 downregulation in the autonomic nervous system
- Carbon monoxide when smoking: When smoked, cannabis smoke contains CO — binds to hemoglobin, reduces oxygen transport. Adds to cardiac stress in addition to the effects of THC. Vaporizers completely eliminate this problem
Cannabis and Heart Attack Risk
The most concerning cardiac finding is the temporal association between cannabis use and myocardial infarction:
- Mittleman et al. 1996 (Circulation): Classic study, N=3,882 heart attack patients. The risk of heart attack was 4.8 times higher in the first hour after cannabis use compared to non-users. The effect is explained by THC-induced tachycardia, increased oxygen demand, and possible vasospastic effects.
- Mechanism — increased myocardial O2 demand: Tachycardia increases the heart’s oxygen consumption. In the presence of pre-existing coronary heart disease (CHD), this increased demand cannot be met → ischemia → infarction. Same mechanism as with extreme physical exertion
- Vasospasm: Cannabis can induce coronary vasospasms (similar to Prinzmetal angina) — especially in young patients without classical coronary heart disease risk factors. Several case reports on cannabis-induced STEMI in individuals under 40 years of age
- Population-based data: Larger studies (Zhang 2022, US registries) show increased cardiovascular event rates among regular cannabis users compared to non-users, even after adjustment for confounding factors
- Relevance for patients with CAD: In patients with known coronary artery disease, angina pectoris or history of myocardial infarction, cannabis use (particularly inhalation) is highly risky. The combination of tachycardia + increased contractility + possible vasospasm can be immediately life-threatening
Atrial fibrillation and cardiac arrhythmias
Cardiac arrhythmias are a less known, but well-documented cardiac complication:
- Atrial fibrillation in young users: Several case series and epidemiological studies show increased incidence of atrial fibrillation in cannabis users, especially in those under 45 years of age. Mechanism: autonomic dysregulation via CB1 on cardiac nerve fibers, increased sympathicotonia
- Bradycardia with chronic use: Paradoxically, long-term users may develop a parasympathicotonic bradycardia after tolerance development — CB1 adaptation on the autonomic nervous system. With concurrent beta-blocker therapy: bradycardia risk is enhanced
- QT prolongation: Individual studies show slight QT prolongation under cannabis — relevant when taken concurrently with QT-prolonging medications (antidepressants, antiarrhythmics, antibiotics). More on this:
Cannabis and Interactions - Cannabinoid Hyperemesis and Cardiology: Severe CINV (Cannabis Hyperemesis Syndrome) with vomiting can cause electrolyte imbalances → arrhythmia triggers independent of direct cannabis effects
Heart Failure: A Special Risk Group
- Reduced cardiac reserve: In heart failure, the heart is unable to meet the increased oxygen demand through tachycardia. Cannabis-induced increase in heart rate can trigger decompensated heart failure
- CO burden in smokers: Patients with heart failure already have reduced oxygen saturation — smoke CO further worsens oxygenation. Absolute contraindication for cannabis smoking in heart failure
- Oral administration as the only option: If cannabis is used medically for heart failure (e.g. pain therapy in comorbidities) — exclusively oral, very low dosage, with close monitoring. Consultation with cardiologists before starting
CBD and the heart: A more favorable profile
CBD behaves cardiovascularly quite differently than THC:
- CBD reduces heart rate and blood pressure: Walsh et al. 2021 (JCI Insight): A single dose of CBD (600 mg) reduced resting heart rate and blood pressure in healthy volunteers. Mechanism: vasodilatory effect, likely via TRPV1 and endothelium-dependent mechanisms
- CBD as cardioprotection: Animal ischemia models show CBD reduces infarct size and improves reperfusion. Antioxidant and anti-inflammatory effects may protect myocardial tissue. Human studies are still missing
- CBD and arrhythmias: Preclinical studies show antiarrhythmic properties of CBD — NF-κB inhibition reduces inflammation in the myocardium as a trigger for arrhythmias. Clinical evidence is limited
- CBD no tachycardia: Unlike THC, CBD does not cause an increase in heart rate — no sympathomimetic effect. For heart patients, CBD is the safer cannabinoid
Who needs to be especially careful
- Coronary heart disease and angina pectoris: Very high risk. Cannabis use, especially inhalation, is contraindicated. In case of medical need: oral CBD under cardiological supervision
- History of heart attack or bypass: Increased risk of re-infarction due to tachycardia and possible vasospasms. Strict contraindication for THC, especially inhaled
- Heart failure: Any form of heart failure increases the risk of cannabis-induced cardiac decompensation. No cannabis without consultation with a cardiologist
- Arrhythmias: Atrial fibrillation, WPW syndrome, Long-QT: Cannabis can trigger or exacerbate arrhythmias. Particularly critical when combined with antiarrhythmic therapy
- Hypertension under treatment: Blood pressure fluctuations caused by cannabis can destabilize the dosage of antihypertensives. Regular monitoring is necessary
- Older patients: Arterial stiffness, reduced cardiac reserve and frequent comorbidities significantly increase the risk. Cannabis in older adults should only be used with geriatric/cardiological supervision.
